Sample Collection Requirements
Submit biopsied embryo cells in unique tubes available from LPL. For multiple embryos, submit a separate sample for each. Ship frozen in special transport containers. A completed Preimplantation Genetic Screening Consent form (Form 23) is mandatory. All samples must be collected and dispatched with prior appointment.
